(The
Service Authorization is used)
Aces$ has developed a Service
Authorization form that must be signed by the Service Facilitator. It will
take the place of the current Service Agreement form for independent
personal support worker paid by Aces$. We will continue to require the
Service Agreement form for all other HBS services.

Who will enroll the Personal Support Workers [PSW]?
(The
Employer, who may ask for assistance from ACES$) Aces$ will enroll the PSW
in their payroll system and for purposes of Medicaid provider enrollment.

Is there a website to see what has been submitted?
(ACES$ will have a web based budget utilization per participant, scheduled
to be in place by 10/1/2007)

Can the PSW fax the timesheets?
(Yes, if faxed, do NOT
send original in addition to faxing it)

What if the PSW quits?
(Employer is responsible for
reporting that to ACES$ and Service Facilitator)
There is a form in the
Enrollment packet for Employers to send to ACES$ for this.

(YES)

Will the employer have to attend the enrollment session?
(It is recommended for the paperwork to be completed properly, but the
Employer can fill out and sign the paperwork and delegate someone to bring
it for them. If the paperwork is not completed properly, the designated
party will need to return it to the Employer for correction – this may delay
enrollment to begin 7/1/2007)

Can a PSW work for 2 consumers?
(Yes, but they need to
fill out PSW Paperwork for each one of the consumer/employer’s separately –
they can only work for one consumer at one given time)

Who will be responsible for submitting the Criminal Record
Checks?
(The Employer submits them to ACES$)

Can the SF bill their 2 hours to ACES$?
(NO)

If the consumer/employer can’t make it, can they send a
representative?
(It is recommended for the paperwork to be completed
properly that the employer attend, but the Employer can fill out and sign
the paperwork and delegate someone to bring it for them. If the paperwork is

In a family where the mother and father are co-guardians can
one be the PSW?
(yes, as long as they are not the employer for the
participant/consumer)

as long as the consumer allows then to – note do NOT send the timesheet if

What is the time lag from the submission of the time sheet
to the Direct Deposit?

be on the pay date as long as the time sheet is not submitted late.

If the timesheet for the end of July is late and later paid
in August, will the July budget be charged?

What happens if the timesheet deadline is missed?

will be processed with the next payroll cycle – remember; you will not see
this on the monthly budget report until the time sheet is processed)

Can a consumer with a guardian be the employer?
(A
consumer with a guardian can be the employer, provided that their guardian
is able to assist them with the responsibilities of directing their own
care. In this case, the guardian would not be eligible to act as the
personal support worker.)
